Affidavit of Residency (of a third party and must be notarized)

Separation Agreement (original and 2 copies)
** Child Support Information Sheet (form CS-47)
** Vital (Certificate of Divorce)

PARTIES WHO HAVE MINOR CHILDREN, THE FOLLOWING IS REQUIRED

** Child Support Obligation Income Statement/Affidavit (both parties)(CS-41)
** Child Support Obligation Guidelines Form (CS-42)
** Child Support Guidelines Notice of Compliance (CS-43)
** Standing Pre-Trial Order ( "Transition in Parenting" Class )
** Child Cope with Divorce/Legal Separation Seminar Certificate (both parties)

Effective January 1, 2004, when any order of child support is entered, the Withholding Order payment of Child Support shall be served immediately, unless the parties have reached an alternative agreement and the Judge approves such agreement. The following forms must be completed and filed with the Withholding Order of Child Support is served immediately:

** 1) Notice to Defendant
** 2) Answer to Order of Withholding

Service fee for Immediate Withholding Order is $20.00 by Sheriff or $5.95 by Certified Mail.

FILING FEE

Court Cost for filing of Divorce is $324.00, payable only by cash, money order or cashier's check. Filing fees are non-refundable.

1. A letter or other document requesting registration.

2. Two (2) copies, including one certified copy, of the determination to be registered.

3. Statement under penalty of perjury that the order has not been modified.

FILING FEE
